MEDIA CREDENTIALS REQUEST FORM


This completed form must be submitted by 5PM (PST) on Monday, November 26, 2007 to receive credentials. Credentials will be held for pick up in the Media Center Office at Mandalay Bay Convention Center, Bayside C for pickup on Friday and Saturday, November 30 and December 1.  Questions, please call (310) 474-5116.  Your credentials are approved unless you hear from us.
